    About the role

    As the CHSP/PAV Response Coordinator, you'll be part of a team responsible for coordinating and delivering services that empower our care recipients to achieve their goals and maintain their independence. You will work directly with clients, their families, and key stakeholders to implement, monitor, and evaluate the CHSP and PAV Response programs while ensuring a person-centered approach to care. Your role will be office based in Shepparton with travelling involved throughout the Hume region.  It will involve a range of duties, including assessing client needs, developing and updating care plans, conducting home visits in an emergency response capacity as well and new client appointments, participating in the after hours on call roster and ensuring compliance with relevant policies and procedures.

     

    About you 

    • Essential Minimum Certificate III in Community Services (or equivalent), including a personal care and first aid competency units.
    • A current Victorian driver’s license, ability to travel through the region
    • Required national police check and Covid x3 vaccinations
    • Strong written, verbal, and numerical communication skills.
    • Proficiency in MS Office and Client Management software (e.g. preferred, Carelink Plus).
    • Experience in care planning, assessments, and working with older adults.
    • A solid understanding of Aged Care Reforms and their impact on service delivery.
    • A commitment to delivering high-quality customer service and building strong, positive relationships with clients and their families with enthusiasm and a positive attitude.
    • Ability to work independently, manage competing demands, and prioritise effectively.
