CHSP Service Coordinator

Your Role in Our Home Care Team
As a CHSP Service Coordinator, you will build genuine relationships with clients, helping them maintain their independence and achieve their goals while remaining safely in their own homes.
Working closely with clients, families, carers, and internal stakeholders, you will coordinate services, conduct reviews, monitor changing needs, and support clients to navigate aged care pathways. This is a rewarding opportunity for a customer-focused professional who is passionate about delivering exceptional care and service coordination.
Key Responsibilities
- Build strong relationships with CHSP clients to support their independence and wellbeing.
- Coordinate services, develop care plans, and conduct annual reviews aligned to client goals.
- Monitor changing client needs and support transitions to additional aged care services where required.
- Maintain accurate client records and ensure compliance with aged care standards and organisational requirements.
- Collaborate with internal teams to deliver high-quality, person-centred outcomes.
- Identify, escalate, and manage risks, incidents, concerns, and complaints appropriately.
- Provide exceptional customer service and proactive client support throughout the care journey.
About You
You are an empathetic and organised professional who thrives on building strong relationships and delivering outstanding customer outcomes.
You will bring:
- Tertiary qualification in Health or a related field
- Experience in aged care, community services, disability services, health, or a related field.
- Strong service coordination or case management experience.
- Excellent communication and relationship-building skills.
- The ability to manage competing priorities and work autonomously.
- Exceptional attention to detail and administrative skills.
- Knowledge of My Aged Care, CHSP, Home Care Packages, or Support at Home programs (highly desirable).
- A commitment to person-centred care and enabling client independence.
- Sound computer skills and experience using client management systems.
