About the Program
Austin Street is a purpose built accommodation service specifically designed to meet the needs of residents requiring complex care due to acquired brain injury. The team works with residents, their families and support staff to provide plans and strategies to improve their wellbeing and independence.
About the Role
VMCH offers an incredible newly created opportunity for a highly skilled Clinical Care Coordinator to join an established team within our Accommodation Services program. You will play a pivotal role in supporting the Manager of Austin Street with a core focus on; sharing your clinical knowledge and expertise to VMCH wider accommodation services programs, provide operational and clinical leadership to a multidisciplinary team, maintain and improve culture and cross-site professional relationships and be an encouraging advocate for change.
You will also work collaboratively with all members of the team including the residents, their families, clinicians, therapists, nurses, support staff, and community agencies to facilitate an effective resident focussed service to people who have significant complex medical and physical support needs resulting from an acquired brain injury.
This is a permanent on-going full-time position working 38 hours per week from Monday to Friday based in our accommodation facilities in Alphington.
Skills & Experience
You will be tertiary qualified in a Bachelor of Nursing and hold a current registration with AHPRA (RN Division 1) and have demonstrated knowledge and understanding in the implementation and delivery of rehabilitation and therapeutic services within disability and rehabilitation. Your exceptional leadership and people skills with a natural ability to listen, learn, influence and build effective relationships with your team will be key to your success.
