About the role
This is an exciting newly created opportunity to work within an established multi-disciplinary team to provide short term restorative care services to clients in the Hume region of Victoria; you will be based in Ballarat or Ararat and travel around the region is required.
As a Clinical Case Manager, you will be allocated a case load and be responsible for proactive, short-term (8 weeks) and long term case management, intake & admissions and assessing and coordinating client care needs to ensure the client receives a collaborative care approach. The approach to working with clients is goal orientated and one of wellness, reablement and capacity building which involves working with clients and their support networks to maximise their independence and quality of life.
You will also utilise your strong leadership experience by supporting, mentoring and leading a small and established multi-disciplinary team including Case Managers and Allied Health practitioners. You may also be required to undertake clinical assessments of consumers who live within the Hume region and assist the Team Leader to develop best-practice approaches for identifying and responding to clinical deterioration.
This is a full-time position and offers flexible ways of working including a combination of outreach across the Ballarat and Grampians Region (including Ballarat and Ararat), work from home and within our offices in Ballarat and/or Ararat.

To be considered for this role, you will be a Tertiary Qualified Registered Nurse and hold a relevant registration with AHPRA. You will have demonstrated experience in case management, assessing and developing care / support plans and have a comprehensive understanding of Aged care frameworks. Furthermore, you will have;
Current and unrestricted registration with AHPRA as a Registered Nurse
