About the role 
 
VMCH has a wonderful opportunity for an NDIS Support Coordinator to join a highly skilled team within the Disability Services space. This is a client-facing role where you will work with NDIS participants to build their resilience, strengthen their capacity and exercise their choice and control in accessing new and relevant support services within the community to reach their goals. You will deliver Support Coordination services to individuals living within the Ararat and Ballarat regions. 
 
This is a permanent full-time position working 38 hours per week and offers flexible ways of working including a combination of outreach, telehealth, working from home and within our offices in Ararat. 
 
Skills & Experience
  • Relevant industry experience and tertiary qualifications in disability or community services
  • Strong knowledge of NDIS, Disability Act and other relevant legislation
  • Demonstrated experience in identifying, measuring and reporting on client and program outcome’s
  • Ability to work autonomously and cooperatively as a member of a team and manage and prioritise competing demands
  • Demonstrated ability to establish and maintain positive professional relationships with stakeholders
  • Use of highly developed verbal communication skills for working with a broad range of stakeholders and written through reports and developed organisational processes
  • Current valid driver’s license and willingness to travel to home visit clients in this regional area.
