About the role
Are you passionate about empowering individuals to achieve their goals and live fulfilling lives? We are seeking a dedicated Support Coordinator to join our Disability Services team in Wangaratta. In this role, you will work collaboratively with participants, families, carers, and service providers to help individuals navigate the NDIS and access the supports they need.
About you
You are an experienced Support Coordinator with strong knowledge of the NDIS. In this role, you’ll work closely with participants to build their capacity, connect them with essential services, and support them in achieving their goals. You will advocate on behalf of clients, develop tailored support plans, and ensure all services meet quality standards and compliance requirements.
Key Responsibilities
- Collaborate with participants, informal supports, and stakeholders to deliver person-centered support.
- Provide coaching and guidance to help participants navigate the NDIS and community resources.
- Organise and facilitate meetings with participants, families, and service providers.
- Monitor budgets and ensure services align with participants’ plans and goals.
- Respond to service gaps or crises, coordinating immediate interventions as needed.
Essential
- Relevant industry experience and a relevant qualification in disability or community services
- Strong knowledge of NDIS, Disability Act and other relevant legislation
- Demonstrated experience in identifying, measuring and reporting on client and program outcome’s
- Ability to work autonomously and cooperatively as a member of a team and manage and prioritise competing demands
- Demonstrated ability to establish and maintain positive professional relationships with stakeholders. Excellent communication skills both written and verbal
- Current NDIS Screening Clearance and Working with Children’s

- Experience working in a NDIS environment
