About the role
As a Care Coordinator, you’ll play a vital role in supporting clients and carers through programmes such as Hoarding & Squalor Assistance, Housing & Care Connections and Care Finder. You’ll ensure quality service delivery, apply consumer-directed care principles, and empower clients to manage their care plans with confidence.
- Conduct comprehensive assessments and develop person-centred care plans.
- Provide advice on service options and coordinate referrals.
- Maintain accurate client records and ensure compliance with funding and legislative requirements.
- Facilitate community engagement activities and represent VMCH at network meetings.
About you
- Tertiary qualification in health or related field (e.g. community service, social work, nursing, allied health).
- Experience in aged care, homelessness, disability, and mental health services
- Strong communication, assessment, and case management skills.
- Understanding of Aged Care Quality Standards and relevant legislation.
- Ability to work independently and manage competing priorities.
- A current Victorian driver’s licence, comprehensive motor vehicle insurance, and current vehicle registration.
- Valid National Police Check.
- Preparedness to travel around Victoria as required
