About the role
As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe, and connected within their own homes and communities. Your days will be filled with meaningful interactions, as you:
- Deliver high-quality, person-centered care to clients in their homes and communities
- Conduct comprehensive assessments, develop care plans, and coordinate services
- Empower clients and carers to make informed choices about their care
- Ensure compliance with aged care quality standards and organisational policies
- Maintain accurate records and uphold client confidentiality
- Work collaboratively with a supportive team and escalate concerns as needed
About you
You are compassionate, adaptable, and genuinely committed to making a positive difference in people’s lives. With strong communication skills and a person-centered approach, you thrive in a role where every day brings new opportunities to support, empower, and connect with others.
Essential
- Tertiary qualification in Health or a related field
- Experience in case management and care planning
- Excellent communication and computer skills
- Understanding of aged care reforms and quality standards
- Ability to work autonomously and manage competing demands
- Commitment to customer service, cultural diversity, and workplace safety
- Willingness to travel as required and a current Victorian driver’s licence
- Satisfactory National Police Check
Desirable
- Knowledge of Carer Recognition Acts and relevant service frameworks (e.g., HCP, CHSP)
- Experience working with people from diverse backgrounds
