About the role

The Case Manager will ensure quality services in accordance with funding body and organisational guidelines to carers/clients in the relevant program and designated regions. The role involves applying consumer-directed care principles to initial enquiries, determining eligibility, conducting initial needs assessments, and identifying resources required within the relevant program. The Case Manager will provide a consistent, high-quality approach to service delivery across regions, maintaining VMCH’s reputation as a leader in the provision of services to support people in the community.

Key responsibilities:

  • Apply person-centered care principles in assessments, care planning, and coordination
  • Empower clients through information, advice, and self-management of their care
  • Develop consumer budgets, ensure accurate data entry, and meet relevant deadlines
  • Manage reviews, provide responses during business hours, and respect client rights
  • Facilitate community engagement and collaborate on service delivery improvements

About you

What you will bring to the role:

  • Must have a Registered Nurse (RN) qualification
  • Previous experience as a care manager, developing care plans and managing consumer budgets
  • Previous experience within the Community services industry
  • Understanding of Aged Care Reforms and Quality Standards
  • Proficiency in MS Office and Client Management applications
  • Customer service focus.
  • Willingness to travel.
  • Driver’s license and police check required.
