About the role

The Commonwealth Home Support Packages (CHSP) Service Coordinator works directly with CHSP care recipients and their support network to identify goals and enable them to maintain and improve their level of independence, empowering them to continue living at home working towards their life goals.  Genuine relationships are developed with eligible CHSP clients through planning and implementing services, annual reviews and regular service coordination.

About you 

  • Tertiary qualification in Health or a related field, with demonstrated experience in case management and care coordination.
  • Strong written, verbal, and numerical communication skills, with proficiency in MS Office and Client Management applications.
  • Experience in developing care plans and conducting client assessments within the aged care sector.
  • In-depth understanding of Aged Care Reforms and their impact on service delivery and client outcomes.
  • Ability to work autonomously, manage time efficiently, and prioritise tasks to meet competing demands.
  • Experience working with Carelink Plus.
