Join Villa Maria Catholic Homes and lead a dedicated team delivering exceptional home care services to our community members. This full-time position offers the opportunity to make a meaningful difference in people's lives while advancing your leadership career in healthcare.
As our Clinical Team Leader, you'll guide and support our home care team in providing compassionate, person-centered care. You'll work closely with clients, families, and healthcare professionals to ensure the highest standards of care delivery while fostering a positive, collaborative team environment.
- Provide clinical leadership and mentorship to home care staff
- Conduct comprehensive client assessments and care planning
- Ensure compliance with regulatory standards and quality frameworks
- Collaborate with families, healthcare professionals, and multidisciplinary teams
- Support staff development through training and professional guidance
- Monitor and evaluate care outcomes to drive continuous improvement
About you
- Experience in home, community and restorative care settings (essential)
- Current AHPRA registration as a Registered Nurse (RN)
- Willingness to travel regionally once per fortnight
- Current Victorian driver’s licence and National Police Check clearance.
- Proven experience in case management, assessment, care planning, and participant budget management.
- Strong written, verbal, and numerical communication skills & proficiency in MS Office.
- Knowledge of Aged Care Reforms, Quality Standards, and community aged care services.
