About the role
As a Clinical Team Leader, you will play a pivotal role in ensuring the delivery of safe, high-quality clinical care to our clients. You will provide structured oversight and support to Clinical Care Partners and non-clinical Care Partners, ensuring best practice, compliance, and continuous improvement. In this leadership position, you will guide and mentor your team, fostering a culture of collaboration, learning, and accountability. Here is what the role involves:
- Supervise and support care partners and allied health staff.
- Conduct supervision meetings, clinical assessments, and participate in MDT meetings.
- Lead End-of-Life and Restorative Care pathways.
- Manage the Clinical Help Desk and escalate concerns as needed.
- Ensure compliance with Aged Care Quality Standards and legislative frameworks.
- Participate in clinical governance, audits, and continuous improvement initiatives.
- Provide or coordinate clinical education and training.
- Build relationships with GPs, hospitals, and allied health providers.
About you
- Tertiary qualification in Nursing and current AHPRA registration as a Registered Nurse (RN) with no conditions.
- Proven experience in case management, especially with complex needs.
- Skilled in assessment, care planning, and participant budget management.
- Strong written, verbal, and numerical communication skills & proficiency in MS Office.
- Knowledge of Aged Care Reforms, Quality Standards, and community aged care services.
- Ability to work independently, manage time, and adapt to change.
- Willingness to travel (regional Victoria and Northern NSW).
- Current Victorian driver’s licence and National Police Check clearance.
