VMCH – At Home Aged Care | Melbourne

VMCH is seeking an experienced Community Physiotherapist to join our At Home Aged Care Allied Health team on a 6‑month fixed term contract.

This is a community‑based role, supporting older people to remain independent and safe in their own homes through high‑quality, client‑centred physiotherapy services.

Key responsibilities

• Deliver physiotherapy assessments and interventions in clients’ homes
• Develop and implement goal‑focused treatment plans
• Support mobility, balance, reablement and overall safety
• Complete professional documentation and reports
• Work collaboratively with clients, carers and internal stakeholders

About you

• Tertiary qualification in Physiotherapy with current general AHPRA registration
• Ability to work independently in a community setting
• Strong communication, organisation and time‑management skills
• Current Victorian driver’s licence and willingness to travel
• Interest or experience in aged care or community health
