About the role 

You will support carers and participants living with a disability to receive the services and support required to maintain their wellbeing so they can live independently and achieve their goals.  This full-time position is located in our modern Mulgrave offices, with onsite parking provided.  The role reports to the Team Leader – Support Coordination.

 

About you

You will be an experienced passionate Support Coordinator with outstanding communication and client service skills allowing you to build strong relationships with a diverse range of people.  You will have excellent time management and organisation skills with the ability to manage competing demands to meet client needs. 

Requirements of the role:

    • Relevant industry experience and tertiary qualifications in Disability or Community Services
    • Knowledge of NDIS, Disability Act and other relevant legislation

 Your responsibilities will include:

    • Providing advice and coordination services to carers / participants to empower choice and self-management to ensure the required services are received
    • Plan and implement services and supports that empower participants to participate in activities.   
    • Promote best practice ‘person centred care’
    • Provide Carer support to address the wellbeing of the carer; introduce carer support services
    • Maintain networks and knowledge of services and liaise with service providers.
    • A Victorian driver’s licence, NDIS Check and a Working With Children’s check
