About the role

We are currently recruiting for a full-time Care Partner based in Mulgrave, supporting clients across the Northern Metro & Eastern Metro region.

As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe, and connected within their own homes and communities. Your days will be filled with meaningful interactions as you:

  • Deliver high-quality, person‑centred care to clients in their homes and communities
  • Conduct comprehensive assessments, develop care plans and coordinate services
  • Empower clients and carers to make informed choices about their care
  • Ensure compliance with aged care quality standards and organisational policies
  • Maintain accurate case notes, documentation and client confidentiality
  • Work collaboratively within a supportive, multidisciplinary team and escalate concerns where required

About you 

You are compassionate, adaptable, and genuinely committed to making a positive difference in people’s lives. With strong communication skills and a person-centered approach, you thrive in a role where every day brings new opportunities to support, empower, and connect with others.

Essential 

  • Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor's degree or above)
  • Experience as a Care Partner or Case Manager within Support at Home, Home Care Packages
  • Excellent communication and computer skills
  • Understanding of aged care reforms and quality standards
  • Ability to work autonomously and manage competing demands
  • Commitment to customer service, cultural diversity, and workplace safety
  • Willingness to travel as required and a current Victorian driver’s licence
  • Satisfactory National Police Check

Desirable

  • Knowledge of Carer Recognition Acts and relevant service frameworks (e.g., HCP, CHSP)
  • Experience working with people from diverse backgrounds
