The position
As a Case Manager – Carer Services and make a meaningful difference in the lives of carers across the community. In this rewarding role, you will partner with carers to assess their needs, develop goal-directed support plans, and connect them with the right services and supports at the right time.
Using a person-centred, strengths-based and capacity-building approach, you will support carers who are caring for a loved one with disability, mental illness, chronic illness or age-related care needs. You will provide tailored advice, advocacy, referrals and short to medium-term interventions that help carers build resilience, feel supported and sustain their wellbeing.

The responsibilities
You are passionate about empowering carers and improving their quality of life. With strong communication, assessment and case management skills, you understand how to support people through complex circumstances with empathy, professionalism and practical guidance.
- Experience in assessment, reassessment and developing goal-directed support plans.
- Provide information and advice to carers to empower them to exercise choice and control, promoting independence, future planning and supporting people in their roles as carers
- Determine priority of Carers needs, escalating situations requiring urgent response
- The ability to work autonomously, manage time effectively and prioritise competing demands.
- A current right to work in Australia, plus the ability to obtain required sector checks including WWCC and NDIS Screening Check.
What’s required
- Relevant tertiary qualifications and/or experience in Community Services, Social Work, Counselling or a related field.
- An understanding of Aged Care and Disability Standards and their application to clients and services.
- Strong written, verbal and numerical communication skills.
- Ability to work autonomously; sound time management skills and ability to prioritise to meet competing demands
- Good computer skills, including Microsoft Office and client management systems
